Innovations Exchange Patient- and Family-Centered Care Video Series

A Patient Advisor’s Perspective on Patient- and Family-Centered Care
This video builds upon the Frontline Innovators series by demonstrating the human impact of a specific patient- and family-centered care (PFCC) innovation that has been adopted by hospitals participating in the Innovations Exchange’s PFCC Learning Community. It offers insights from a former patient—and current patient advisor—illustrating the value of hospitals partnering with patients to advance PFCC. The video is particularly useful for hospitals interested in starting a patient and family advisor program, as well as patients and families who are considering becoming advisors.
Frontline Innovators on Providing Patient- and Family-Centered Care
These three videos feature brief profiles of health care professionals sharing stories about how they have put the core concepts of patient- and family-centered care into practice. Stories highlighted in the videos explain how current and former patients and families advise a hospital on care improvement, how a day-treatment program partners with parents to support and stabilize children with dual psychiatric and chronic disease diagnoses, and how a hospital brings together the expertise of families and geriatric-trained staff to improve outcomes in a separate, seniors-only emergency department. These videos and their related profiles show how innovators are working in partnership with patients and families to improve care.
