• Pathology: percentage of biopsy and cytology specimen reports with a diagnosis of non small cell lung cancer that are classified into specific histologic type or classified as NSCLC-NOS with an explanation included in the pathology report.

• Pathology: percentage of breast cancer resection pathology reports that include the pT category (primary tumor), the pN category (regional lymph nodes) and the histologic grade. This updates a previously published measure summary.

• Pathology: percentage of colon and rectum cancer resection pathology reports that include the pT category (primary tumor), the pN category (regional lymph nodes) and the histologic grade. This updates a previously published measure summary.

• Pathology: percentage of pathology reports for primary lung carcinoma resection specimens that include the pT category, pN category and for non small cell lung cancer, histologic type (squamous cell carcinoma, adenocarcinoma and not NSCLC-NOS).

• Pathology: percentage of patients with esophageal biopsy reports for Barrett's esophagus that contain a statement about dysplasia.

• Pathology: percentage of patients with quantitative breast tumor human epidermal growth factor receptor 2 (HER2) immunohistochemistry (IHC) evaluation using the ASCO/CAP recommended manual system or a computer-assisted system consistent with the optimal algorithm for HER2 testing as described in the current ASCO/CAP guidelines.

• Pathology: percentage of primary malignant cutaneous melanoma pathology reports that include the pT category and a statement on thickness and ulceration and for pT1, mitotic rate.

• Pathology: percentage of radical prostatectomy pathology reports that include the pT category, the pN category, the Gleason score and a statement about margin status.
