Improving safety for hospitalized patients: much progress but many challenges remain.
Some improvement has been achieved in the 15 years since To Err Is Human was published, though progress has been deemed slower than anticipated. This commentary reviews key areas that demonstrate what has been accomplished in patient safety such as reductions incatheter-associated urinary tract infections and highlights efforts led by the Partnership for Patients program.
