Highly reliable care is a goal for harm reduction efforts. This commentary discusses the importance of understanding high reliability principles to strengthen patient safety efforts. The authors suggest determining the cost impact of preventable adverse events in conjunction with nurse-sensitive patient outcomes as an improvement metric.
