Opportunities for Shared Savings Program ACOs to Get Involved
Thank you for attending the “Opportunities for Medicare Shared Savings Program (MSSP ACOs) to Get Involved” webinar. To access the webinar materials, please visit http://go.cms.gov/mdpp and scroll down to the MDPP Orientation section and look for “Webinar: Opportunities for Medicare Shared Savings Program (MSSP) Accountable Care Organizations (ACO) to Get Involved”.
As discussed during the webinar, diabetes affects many individuals and effective behavior change can reduce the risk of type 2 diabetes. The Medicare Diabetes Prevention Program (MDPP) is a group-based intervention targeting at-risk Medicare beneficiaries. Medicare pays organizations, called MDPP suppliers, to furnish a group-based intervention to at-risk Medicare beneficiaries, using a CDC-approved National Diabetes Prevention Program curriculum.
Benefits of MDPP to ACOs
- If enrolled as an MDPP supplier, MDPP’s value-based payment schedule pays suppliers for furnishing MDPP services.
- MDPP aligns with ACO goals by reaching a high-risk population, promoting healthier behaviors, and promoting cost-savings through value-based care.
Pathways to Get Involved
Educate patients and providers on prediabetes and available preventive services.
- Promote awareness of prediabetes and prevention. Encourage ACO providers to educate at-risk patients about prediabetes and lifestyle changes to improve health.
- Screen, test, and refer patients. Help determine eligibility by screening Medicare patients for prediabetes. Refer eligible patients to local suppliers is the Supplier Map.
Become an MDPP supplier and/or partner with nearby organizations
- Enroll as an MDPP supplier to furnish MDPP services. Note that your organization must first obtain preliminary or full CDC Diabetes Prevention Recognition Program (DPRP) recognition before enrolling in MDPP.
- If your organization already has preliminary or full DPRP recognition, enroll online through the Provider Enrollment Chain and Ownership System (PECOS) or review the enrollment paper application.
- Visit the MDPP website for more information on enrolling as an MDPP supplier:
- See the Enrollment Fact Sheet to understand the Medicare enrollment process.
- Review the Supplier Requirements Checklist to confirm your organization meets the requirements needed to enroll.
- Use the Coach Eligibility Fact Sheet to confirm that your coaches meet eligibility requirements and obtain an NPI.
- Watch the Enrollment Video to walk through the enrollment process step-by-step in PECOS.
- Review the Enrollment Process Timeline to know what to expect after your application is submitted
- Partner with a National DPP organization. Explore partnership opportunities with CDC DPRP recognition delivery organizations in your areas listed in the CDC’s Registry of Recognized Organizations.
