SNF Trigger Tool: Measuring Patient Harm in Skilled Nursing Facilities.

Trigger tools are a way to uncover weaknesses in care processes and monitor the incidence of those problems. This six-session webinar will educate participants about how to utilize atrigger tool developed to support team engagement and use of data to track the impact of safety efforts in skilled nursing facilities.
