Accuracy of the Safer Dx Instrument to identify diagnostic errors in primary care.

Diagnostic errors are difficult to detect from medical records. This study describes a structured tool that assists in physician review of medical records in order to determine if a diagnostic error was present. Compared to the researchers' prior open chart review method, this tool provided adequate accuracy to help identify diagnostic errors.
