This report compiles patient safety data documented by Massachusetts hospitals. The latest numbers represent a substantial increase in the serious reportable events recorded, from 444 the previous year to 753. Updated criteria to include different types of incidents to report and the implementation of an electronic reporting system may have contributed to this change.
