Nominations for After Visit Summary Commissioned Paper Due December 4th
With funding from the Agency for Healthcare Research and Quality (AHRQ), the Institute of Medicine (IOM) Roundtable on Health Literacy is producing a paper on implementing after visit summaries. The IOM is seeking nominations of individuals with expertise to prepare a commissioned paper.The selected author will receive $10,000 for successful completion of the task which is described below.
Research suggests that 40-80 percent of medical information provided in the doctor’s office is completely forgotten by the time the patient gets home and half of the information is recalled incorrectly (Kessels, 2003). Federal meaningful use requirements dictate that a summary be created after every primary care visit and provided in either written or electronic format and specifies elements to include. However, after visit summaries (AVS) are often written beyond the comprehension level of patients, are lengthy, contain irrelevant information, and research has shown repeatedly that many patients do not understand the instructions they receive. The Agency for Healthcare Research and Quality (AHRQ) has awarded a grant that will develop and refine a patient-centered electronic AVS that is easy for clinicians to use and contains patient-centered information that is understandable and actionable by patients and caregivers, including those with low health literacy.
Rather than duplicate the efforts of the ongoing project described above, the purpose of the commissioned paper is to explore what it will take to ensure widespread use in primary care of an effective after visit summary as a tool for patient education and engagement.
