Nurse practitioner–led medication reconciliation in critical access hospitals.

This pre-post study demonstrated that nurse practitioner–led medication reconciliation was effective in reducing medication discrepancies among patients discharged from hospital to home. This finding adds to theevidence that risks associated with hospital discharge can be mitigated by health care teams.
