Primary care medication safety surveillance with integrated primary and secondary care electronic health records: a cross-sectional study.

This cross-sectional study of patient records found that linked primary and secondary care data provides a more robust surveillance of medication safety. Older patients and those prescribed multiple medicationswere at highest risk for prescribing hazards.
