Public comment for Post-Acute Care Potentially Preventable Hospital Readmission Measures

CMS is seeking public comment on the Development of Potentially Preventable Hospital Readmission Measures for Post-Acute Care.

The Call for Public Comment period opened on November 2 and will close on November 16, 2015. This public comment period is open to any interested parties who would like to provide input on these measures under development.
