Summary
Through Project Bridge, Miriam Hospital links HIV-positive prisoners to community-based medical care and social services to enable successful reentry into the community. Beginning a few months before release and continuing for 18 months afterward, participants receive intensive case management from a professional social worker and outreach worker, including an individualized care plan; assistance in accessing needed medical and mental health care, substance abuse treatment, and social services; and ongoing monitoring and followup. The program has significantly enhanced access to medical, mental health, and social services, and reduced recidivism.

Moderate: The evidence consists of post-implementation data on the percentage of participants receiving medical, mental health, substance abuse, and social services, along with a comparison of reincarceration rates between participants and nonparticipants.
