This commentary provides an overview of the findings from To Err Is Human and Crossing the Quality Chasm—which launched 15 years of focus on systems-oriented approaches to patient safety—and reviews recommendations from Improving Diagnosis in Health Care in the context of risk management to provide guidance in how risk managers can help reduce diagnostic errors.
