New Report on Patient Safety in Primary Care
A new report from AHRQ offer results of an environmental scan of evidence-based strategies to improve patient safety in primary care settings by engaging patients and families, and describes factors leading to patient harm in primary care settings, including patient, clinician, and health system. The State of Patient Safety in Primary Care Environmental Scan report pinpoints four key threats to patient safety in primary care, and reveals that while the field of patient safety in primary care is new, several important interventions exist that focus on providers, patients, and practices as target audiences. The report also reviews engagement strategies aimed at reducing the impact of breakdowns in communication, care transitions and environment of care, medication management, diagnosis and treatment, and presents recommendations for clinical practice and future research. Also available are a compendium of interventions based on the results of the environmental scan. The complete AHRQ Guide to Improving Patient Safety in Primary Care Settings by Engaging Patients and Families (that will include the interventions and implementation guidance) will be available in 2018.
