• Hypertension diagnosis and treatment: percentage of adult patients age greater than or equal to 18 years diagnosed with chronic kidney disease whose blood pressure is at SBP less than 140 mmHg and DBP less than 90 mmHg.

• Hypertension diagnosis and treatment: percentage of adult patients age greater than or equal to 18 years diagnosed with diabetes whose blood pressure is at SBP less than 140 mmHg and DBP less than 90 mmHg.This updates a previously published measure summary.

• Hypertension diagnosis and treatment: percentage of adult patients age greater than or equal to 18 years diagnosed with hypertension who are not at goal for hypertension and have received counseling on diet and physical activity in the past 12 months.

• Hypertension diagnosis and treatment: percentage of adult patients age less than 60 years diagnosed with hypertension whose blood pressure is at SBP less than 140 mmHg and DBP less than 90 mmHg. This updates a previously published measure summary.

• Hypertension diagnosis and treatment: percentage of patients age greater than or equal to 60 years diagnosed with hypertension whose blood pressure is at SBP less than 150 mmHg and DBP less than 90 mmHg.
