Comprehensive ESRD Care Initiative, Request for Application (RFA)

Center for Medicare and Medicaid Innovation (CMMI), will host a Special Open Door Forum (ODF) to allow dialysis facilities, nephrologists, other Medicare providers of services, suppliers, and other interested parties to ask questions on the revisions to the Request for Application (RFA) for the Comprehensive ESRD Care Initiative that was released on April 15, 2014.
The purpose of the Comprehensive ESRD Care (CEC) Initiative is to improve outcomes for Medicare beneficiaries with end-stage renal disease (ESRD) and reduce total per capita expenditures by creating financial incentives for dialysis facilities, nephrologists, and other Medicare providers of services and suppliers to collaboratively and comprehensively address the extensive needs of the complex ESRD beneficiary population. CMS has worked with stakeholders to revise the CEC initiative to allow for broad participation by both large dialysis organizations (LDOs) and non-LDOs. The changes reflect revisions to the participant owner requirements, the financial risk arrangement, and additional options for non- LDO’s aggregation for the purpose of financial calculations. The revisions are intended to address concerns on financial arrangements, non-LDO’s option to satisfy the requirement that an ESCO have a minimum of 350 matched beneficiaries for financial purposes while providing non-LDOs the opportunity to collectively lower minimum savings rate and increase financial incentives under the Initiative.

Ambulance Open Door Forum
Overview:
The Ambulance Open Door Forum (ODF) addresses issues related to the payment, billing, coverage and delivery of services in the ambulance industry. The Ambulance Fee Schedule rules, rural and other ambulance payment enhancements, requirements for ambulance service certification and payment determinations, are just some of the many types of issues addressed within the forum. In addition, discussions differentiating the rules related to provider-based and independent ambulance services are facilitated. Timely announcements and clarifications regarding important rulemaking, agency program initiatives and other related areas are also included in the forums.
