Improving Adult Vaccination Practices Toolkit
Description
This toolkit provides strategies and templates to help clinicians and other health care professionals increase vaccination rates for influenza and pneumococcal disease in high-risk and older adults, in order to reduce the risk of disease outbreaks, serious illness, hospitalization, and deaths, as well as related medical costs.
The toolkit includes the following components:
- Algorithm and instructions for toolkit implementation
- Sample data collection form and quality measures/calculation instructions
- PowerPoint slides available for interventions
- List of resources available for interventions
- Sample action plan template
