Immunizations: percentage of patients or parents (if patient younger than 18 years) who receive education regarding the importance of immunizations and recommended immunization schedules.
Immunizations: percentage of patients who by age 13 years were up-to-date with the following recommended adolescent immunizations: one human papillomavirus vaccine (HPV) (for females), one meningococcal (MCV4), one tetanus, diphtheria toxoids and acellular pertussis vaccine (Tdap), and one influenza within the last year.
Immunizations: percentage of patients, 19 years and older, who are up-to-date with the following recommended immunizations: one diphtheria toxoids vaccine (Td) in the last 10 years, two doses of varicella or history of disease up to year 1995, pneumococcal polysaccharide vaccine (PPSV23) for patients 65 years and older, one influenza within the last year, and herpes zoster/shingles for patients 60 years and older.
