• Comprehensive adult diabetes care: percentage of patients 18 to 75 years of age with type 1 or type 2 diabetes who had a foot exam (visual inspection with either a sensory exam or a pulse exam) during the measurement year.

• Comprehensive adult diabetes care: percentage of patients 18 to 75 years of age with type 1 or type 2 diabetes who had an eye exam (retinal) performed.

• Comprehensive adult diabetes care: percentage of patients 18 to 75 years of age with type 1 or type 2 diabetes who received medical attention for nephropathy.

• Comprehensive adult diabetes care: percentage of patients 18 to 75 years of age with type 1 or type 2 diabetes whose most recent blood pressure reading is less than 140/90 mm Hg (controlled).

• Comprehensive adult diabetes care: percentage of patients 18 to 75 years of age with type 1 or type 2 diabetes whose most recent hemoglobin A1c (HbA1c) level is greater than 9.0% (poorly controlled).

• Comprehensive adult diabetes care: percentage of patients 18 to 75 years of age with type 1 or type 2 diabetes whose most recent hemoglobin A1c (HbA1c) level is less than 8.0% (controlled).

• Comprehensive adult diabetes care: percentage of patients 18 to 75 years of age with type 1 or type 2 diabetes whose most recent LDL-C level is less than 100 mg/dL.

• Disease-modifying anti-rheumatic drug therapy for rheumatoid arthritis: percentage of patients who were diagnosed with rheumatoid arthritis and who were dispensed at least one ambulatory prescription for a disease modifying anti-rheumatic drug (DMARD).

• Osteoporosis management in women who had a fracture: percentage of women 67 years of age and older who suffered a fracture and who had either a bone mineral density (BMD) test or medication to treat or prevent osteoporosis in the six months after the date of fracture.

• Use of imaging studies for low back pain: percentage of patients with a primary diagnosis of low back pain who did not have an imaging study (plain x-ray, MRI, CT scan) within 28 days of the diagnosis.
