CHIPRA National Evaluation Site Posts Quality Demonstration Grantee Resource
Many reports and other types of resources from the States participating in the National Evaluation of the CHIPRA demonstration grant program have been posted to the National Evaluation Web pages. These resources provide additional details about the States’ projects and lessons learned that may guide others interested in pursuing similar initiatives. Some of these resources include:
- Evaluations of initiatives in Maine to improve immunization and screening rates.
- A compilation of best practices from the Massachusetts Medical Home Learning Collaborative and a toolkit to aid in the patient-centered medical home certification process.
- A framework from Massachusetts of key elements involved in care coordination and associated measures to monitor the effectiveness of care coordination.
- A video series from North Carolina highlighting issues faced by adolescent patients and their providers that models how providers can engage with patients to address those issues.
