A prospective cohort study of medication reconciliation using pharmacy technicians in the emergency department to reduce medication errors among admitted patients.

Medication reconciliation performed by clinical pharmacists in emergency departments has been shown to improve admission medication regimen accuracy. This prospective cohort study examined whether a similar effect may be seen with pharmacy technicians reviewing medications prior to admission. However, the program did not result in any significant reductions in medication errors.
