2015 CAHPS® Health Plan Survey Database Submission Now Open

CAHPS^® Health Plan Survey Database Submission Now Open

CAHPS 5.0/5.0H Health Plan Survey users who administered the survey from October 2014 - June 2015 can submit their data to the CAHPS Database June 1 - 12.

Data collected from surveys administered from October 2014 - June 2015 will be accepted for the following survey versions:

• Adult Medicaid Questionnaire
• Child Medicaid (or CHIP) Questionnaire without the Children with Chronic Conditions Item Set
• Child Medicaid (or CHIP) Questionnaire with the Children with Chronic Conditions Item Set

The submission requirements, Data File Specifications, and Data Use Agreement (DUA) are available at: https://cahpsdatabase.ahrq.gov/HPDSS/login.aspx. All required items must be submitted no later than June 12.
