Inaccurate transfer of clinical information during handoffs and signouts has been linked to adverse clinical events within hospitals from the emergency department to the intensive care unit, according to a patient safety primer available on AHRQ’s Patient Safety Network (PSNet). The primer introduces concepts that can be used to implement structured handoff and signout procedures. It also identifies existing guidelines and innovative strategies for the handoff process.
