The target audience for this learning activity includes general medicine practitioners, specialists (e.g., chronic and infectious disease specialists, gerontologists, oncologists, and emergency department and intensive care unit personnel), pharmacists, nurses, nurse practitioners, physician assistants, medical assistants, case managers, health educators, health care administrators, and policymakers.

Overall Learning Purpose/Goal

The goal of the Webinar is to present information to a variety of health care professionals about available evidence regarding the effect of medication reconciliation on clinically significant discrepancies and hospital readmission, and to provide information about the MATCH toolkit.

Learning Objectives

At the conclusion of this activity, participants will be able to:
  1. Define medication reconciliation as formal patient safety practice.
  2. Identify four sources of medication information.
  3. Describe what is known about potential harms associated with unintended medication changes at care transitions.
  4. Describe the steps outlined in the MATCH toolkit for evaluating, designing, and implementing medication reconciliation processes.

    For Pharmacists: Provide accurate and appropriate counsel as part of the treatment team.
    For Registered Nurses: Provide appropriate care and counsel for patients and their families.

Janice L. Kwan, MD, MPH, FRCPC
Division of General Internal Medicine, Mount Sinai Hospital, University of Toronto
Dr. Janice Kwan is a general internist at Mount Sinai Hospital in Toronto. She is a Master of Public Health candidate in health policy and management at Harvard University and a Veterans Affairs Quality Scholars fellow at the University of Toronto. Her research focuses on health systems reform and quality improvement. Her work has appeared in the Annals of Internal Medicine and the British Medical Journal. She completed her undergraduate and medical degrees at the University of Toronto, and was Chief Medical Resident at Mount Sinai Hospital. Dr. Kwan has no financial relationships to disclose.
