A Patient Safety Rounds pilot program at clinics affiliated with a large research and education institution.

This study describes a pilot program to introduce patient safety rounds at a group of outpatient clinics. These rounds identified many patient safety issues and were viewed as a success by participants, but the program was discontinued following the departure of key leadership.
