- Emergency department (ED): admit decision time to ED departure time for admitted patients. This updates a previously published measure summary.
- Emergency department (ED): median time from ED arrival to ED departure for admitted ED patients. This updates a previously published measure summary.
- Heart failure: percent of patients discharged home with written instructions or educational material given to patient or caregiver at discharge or during the hospital stay addressing all of the following: activity level, diet, discharge medications, follow-up appointment, weight monitoring, and what to do if symptoms worsen. This updates a previously published measure summary.
- Heart failure: percent of patients with documentation in the hospital record that LVS function was evaluated before arrival, during hospitalization, or is planned for after discharge. This updates a previously published measure summary.
