Help Improve SAMHSA's Disaster Services and Products in 15 Minutes
While the number of major disaster declarations varies from year to year, the need to be prepared and able to respond to a disaster or emergency does not. You recently received an email invitation to participate in a web survey to evaluate the services provided by the SAMHSA Disaster Technical Assistance Center (DTAC). If you have not had a chance to complete the survey yet, please click on the link below or copy and paste it into your browser. It only takes about 15 minutes to complete.
We want to hear your thoughts about SAMHSA DTAC services and products so that we can continue to provide you with high-quality customer service whether you receive a major disaster declaration or need to respond to a smaller emergency in your community.
Please complete the survey by April 28, 2015.
Major Disaster Declarations By Year

You have been selected to participate in this survey because you have either requested technical assistance from SAMHSA DTAC in the past or you are subscribed to the DTAC Bulletin or The Dialogue.
If you have any questions about the survey or technical difficulties, please contact us at 1-800-305-3515 or emaildtac@samhsa.hhs.gov.
Thank you for your participation!
